Your first appointment with Prue is most valuable when a few key details are in place beforehand. The steps below will guide you through what to prepare.
Booking Your First Appointment
Step One
You will need a current referral from your GP or specialist to see Prue. A valid referral also allows you to claim a Medicare rebate on the consultation fee.
Ask your GP for a referral
Step Two
If you haven’t already had recent investigations, ask your GP to arrange them. If you have, ask for copies of results to be sent to the clinic before your appointment.
For women:
Blood tests: Rubella, Varicella, Thyroid function, Prolactin, Vitamin D, Ferritin, Day 21 Progesterone (if your cycle is regular), and AMH if you’re over 35
Genetic carrier screen results if completed
Pelvic ultrasound
For men:
Semen analysis
Any other pathology, radiology results, or correspondence you think may be relevant are also welcome.
Arrange the relevant investigations
Step Three
Once your appointment is booked, you will receive a new patient form. Please complete it and return to geelong.admin@monashivf.com ahead of your visit.

Photo ID
Any test results or correspondence not already sent through
